ADMISSIONS

 A brochure, price information and an application for enrollment are available at the Children’s Village office.

 A completed application may be mailed or delivered to the Children’s Village office.  A non-refundable registration will be collected with the application.

 The following items must be received before the child’s first day:

  • Physician’s form and immunization record
  • Emergency information and notification form
  • Health history – Parent’s report
  • Getting Acquainted form
