A completed application may be mailed or delivered to the Children's Village office. A non-refundable registration will be collected with the application.

	Parent Handbook & Admission Agreement
	Child's Preadmission Health Evaluation
	Immunization record
	Child's Preadmission Health History
	Signed & Dated "Notice - Parent's Rights" Receipt
	Student Enrollment Record & Consent for Medical Treatment
	Personal Rights - Day Care Facilities
	Suncreen Permission
